The Rehabilitation Process

 

Working Together For Your Recovery

A comprehensive rehabilitation program involves many services from an interdisciplinary team of professionals with the expertise to fully support your treatment. Our experienced team will provide the innovative care you need to improve your health and daily functioning.

 

ADMISSION

After being welcomed by the staff , we’ll help you get familiar with your new surroundings. This may include a tour of our campus, settling in to your room if you will be staying with us, meeting the staff , and helping us get to know you. A nurse will assess what your medical and personal care needs will be. You and your family or guardian will review and complete your admission paperwork.

INITIAL ASSESSMENT

You’ll meet with different members of your treatment team to kick off your rehabilitation, which typically includes:

  • Physical Therapy

  • Occupational Therapy

  • Speech and Language Pathology

  • Recreational Therapy

  • Social Work

  • Psychology

  • Case Management

  • Nursing

  • Respiratory Therapy

  • Dietary & Nutritional Services

  • Other Staff

Our team is excited to get to know you, learn about your goals, and assess your needs in their particular treatment area.

TREATMENT PLAN

To begin, we’ll sit down with you, your family, and the treatment team at a Care Conference—a meeting held to review your individual rehabilitation goals. The team will ask about your transition and discharge plans, and will work closely with you and your supporters to develop a rehabilitation plan that establishes the necessary steps to achieve your goals.

YOUR THERAPY SCHEDULE

Your physician and treatment team will determine the type, frequency, and duration of your therapy sessions which are held Monday through Friday, typically between 8:00 am and 5:00 pm.

TRANSITION AND DISCHARGE

To ensure we maximize the resources available to you during your stay, we’ll begin with the end in mind, talking through your options for transitioning to the next stage of your recovery. The length of your treatment depends upon your specific needs. Many clients return to home or work following rehabilitation, while others transition to new living environments or to less intensive rehabilitation programs within the Special Tree system.

Follow up resources are available for all clients and their circles of support as well as throughout the rehabilitation process.
